What is a Safety Audit?

A Safety Audit is a review of your records to confirm that your company has basic safety management controls in place.

In other words, FMCSA wants to see that you have a system in place, not just paperwork.

This includes compliance with:

• Federal Motor Carrier Safety Regulations (FMCSRs) • Hazardous Materials Regulations (HMRs) • Recordkeeping and operational control requirements

How the audit is conducted

The audit is not always conducted in person.


It can be completed:

• At your place of business • Electronically • By submitting documents via mail or fax

FMCSA will notify you of the type of audit you’ve been selected for.

During the process, you will be asked to provide documentation that demonstrates you have control over your operation.

How FMCSA evaluates your company

The auditor is not looking for perfection.

They are looking for clear signs of:

• Organization • Consistency • Internal control

This is why everything comes down to three key areas:

• Drivers • Vehicles • Company compliance

Drivers: where most operations fall short

This is where most issues come up.

It’s not just about having documents. It’s about consistency and alignment across your operation.

FMCSA evaluates whether:

• Your drivers are properly licensed • Their driving records are reviewed regularly • Their medical certificates are up to date • Your hours of service records are consistent • There is supporting documentation behind your logs

FMCSA may cross-check logs with receipts, trips, and operational records to verify accuracy.

Vehicles: a direct reflection of your operational control

Your fleet should reflect organization and proper maintenance.

FMCSA reviews whether:

• You have all units clearly identified • You know exactly what you are operating • Inspections are up to date • There is a system in place for maintenance

Key points:

• Every unit must have an annual inspection • In combinations, each component must be inspected • If you transport hazardous materials, documentation must be complete

Compliance: where the audit is won or lost

This is the most critical area.

FMCSA evaluates whether your company operates with structure.

They review:

• Adequate Proof of Insurance • Active Drug and Alcohol Program • Random testing procedures • Accident register when applicable • Consistent internal processes

At this stage, you’re not just presenting documents. You’re demonstrating how your business operates.

What can cause an automatic failure

There are violations that can result in an immediate failure of the Safety Audit.

Some of the most critical include:

• Using a driver without a valid CDL • Using a disqualified or medically unqualified driver • Not having a Drug and Alcohol Program • Not having a random testing program • Using a driver who failed or refused a test • Operating without the required level of insurance • Failing to maintain hours of service records • Operating vehicles declared out of service before repairs • Failing to perform required annual inspections

These are not minor issues. FMCSA considers them serious violations.
